  Eligible for the Cafeteria Plan.
  $10,000 is provided by the Board at a monthly cost of $1.70 (no cost to the employee).
  Employee may purchase additional life insurance as follows:
   $10,000 - $2.20
   $20,000 - $4.40
   $30,000 - $6.60
   $40,000 - $8.80
  Dependent life may be purchased for a total cost of $2.27 which provides:
   $10,000 on Spouse
   $5,000 on each Dependent
(Must purchase additional life on employee in order to purchase dependent life.)
