Background Check Procedure

In the interest of student and staff safety, we will screen in accordance with the following guidelines. ALL parents/guardians chaperoning field trips or using school transportation must successfully complete the background check screening process. This service is free of charge. Background checks are valid for the duration of the current school year and will need to be renewed yearly.

How to complete a background check:

1. There are two ways to retrieve the forms needed for a field trip. A parent/guardian may request the forms from the front office of the school respective to the location that the child attends, or they may access the forms by clicking the link below. The forms needed to complete this process include the Consent/Inquiry Form and the Child Privacy Act.

  • Please note that if you have children that attend different schools within the school district, you will only need to complete the forms once.

2. Parent/Guardian(s) will need to complete the sections highlighted in yellow on the Consent/Inquiry form and fill out the information on the back of the Child Privacy Act. When completed, the school’s secretary or a Notary will need to notarize the Consent/Inquiry Form.

3. Parent/Guardian(s) will then take the forms to the Sheriff’s Department to complete the background check. The Sheriff Department’s hours are from 8:00am to 5:00pm.

4. After the background check is completed, Parent/Guardian(s) will then bring the forms to the HR Department at the district office. DO NOT return the completed forms to the school.

5. In the event that the HR department is unavailable, results can be left with the receptionist in a sealed envelope. Envelopes will be provided in the event that one is needed.

  •  Please include last name, school(s), and a phone number on the envelope.

6. The HR department will then review the background checks. Each Schools’ list of approved chaperones will be updated.

  •  If the background check is unfavorable, the principal will be notified by Human Resources.
  •  Findings of an offense shall not immediately disqualify a parent from participating in a field trip.
