Student Checkout: Regardless of the student's age, a note from a parent or guardian must be presented for early dismissal from school. The note should include
- the student's name
- time of dismissal
- reason for dismissal
- parent or guardian's signature and
- a telephone number where the parent or guardian can be reached.
This note must be presented at the front office before checkout, and the student will receive a dismissal slip (blue slip). Students leaving early must sign out in the attendance office. To exit, students who drive must present the signed dismissal slip at the CCHS Welcome Center. Students who do not drive must be signed out from the attendance office by a parent or guardian listed in the student directory. No student checkouts are permitted after 2:45 PM. Parents or guardians must come into the attendance office to check students out, show a valid ID, and be verified on the student's checkout list in Infinite Campus.
If a parent or guardian calls to request an early dismissal, an email to cchsattendance@colquitt.k12.ga.us or fax must follow, including the student's name, time of dismissal, reason for dismissal, parent or guardian's signature, and telephone number, along with the name of the person picking up the student if it is not the parent/guardian. The email or fax must match the contact information in Infinite Campus. Phone calls alone are unacceptable for checkout, and verbal phone call releases will not be accepted. Parents or guardians must submit written documentation verified by the attendance office before checkout. Failure to submit a parent note will require the parent or guardian to check the student out in person through the attendance office.
Students will not be called to the attendance office for checkout until the parent or guardian arrives in person or a parent note is confirmed. Please do not call or text students during class to come to the office.
College Visitation Form Process
- Student must take a copy of the “College Visitation Certification” Form with them on their college visit. This may be picked up from Counselors’ Corner or printed from the CCHS Guidance website.
- Students will visit college and have college representative complete the form before leaving the campus.
- Students will turn in completed form to attendance clerk. After form is turned in, the clerk will change the unexcused absence to a “college visit”.
